Job Description

This position will provide professional nursing care and case management support to patients with neuromuscular disorders as part of a multidisciplinary team.

Preferred Qualifications

Previous experience caring for patients with neurological conditions. Bilingual, English/Spanish, with the ability to speak, read and write Spanish.

Job Duties

Job Duty

Coordinate continuity of care for neuromuscular patients followed as outpatients. Provide patient and family education on treatments and the importance of following the neuromuscular team’s treatment plan.

Job Duty

Manage clinic schedules to ensure that they are full by reviewing the wait list and scheduling patients as appointment slots become available. Ensure patient questionnaires are completed prior to patient appointments. Provide clinical schedules to MDA representative, ensuring authorizations are documented in patient records.

Job Duty

Assist with the completion of FMLA, disability paperwork, and appeals. Draft letters of medical necessity and correspondence requested by patients and their families.

Job Duty

Assist with coordinating referrals to specialists for PEG/J-tube insertion, Myobloc injections, and IV/intrathecal therapies. Ensure that prescriptions/orders for DME are processed. Oversee scheduling of diagnostic services, procedures, treatments, and hospital admissions.

Job Duty

Provide ongoing support and guidance to patients and their families throughout the disease process in order to enhance quality of life and self-esteem. Maintain knowledge of community resources, government services, insurance reimbursement practices, and support groups. Serve as an advocate for patients in dealing with healthcare providers, insurance plans, and community resources.
